Thesis

Over 15 million people in the UK suffer from long-term health conditions (LTCs), such as diabetes, asthma, and heart disease, with most experiencing poorer health outcomes and reduced quality of life as a result. There is a high demand for access to larger scale specialist support services, not only to improve poor wellbeing and health outcomes, but to decrease the utilisation of direct health care services such as the NHS.

 

Investment

We are supporting Healthier Lives Together, a social prescribing programme working to create sustained lifestyle changes and improved selfcare for people living with long-term health conditions. The project is commisioners by the NHS North East Lincolnshire andThe National Lottery Community Fund, with project finance and project management support from Bridges.

 

Target Outcomes

Healthy Lives Together aims for improved well-being, improved health, and reduced utilisation of health care services for those with long-term health conditions. So far, 339 people have completed the programme since its launch in 2018. The programme aims to create an exemplar model that can be used as a pilot for future social prescribing outcomes contracts.
