Many people within the UK are suffering from long-term health conditions, such as diabetes, asthma, and heart disease. Treatment for diabetes accounts for about 10% of the NHS budget, spread between primary and secondary care. With an increasing pressure on the NHS, there is a strong need for access to larger scale specialist support services, to improve poor wellbeing and health outcomes.

 

Investment

We are supporting Healthier Living Partnerships, a two-year programme for those most at risk of developing Type 2 Diabetes in Devon. This contract was designed by Devon County Council, drawing on advice from KPMG that was provided through a development grant (put in place to catalyse social outcomes contracts in the UK) from the National Lottery Community Fund. Bridges partnered with the Council to co-commission the supply chain to deliver the targeted outcomes.

 

Target Outcomes

Healthier Living Partnerships aims for sustained healthy behaviour change and improved ability to self-care, resulting in a reduction in incidence and complications of diabetes, improved wellbeing, improved general health, and reduced utilisation of healthcare services. It is the first diabetes prevention programme in the UK to be funded by a local authority on an outcomes basis. So far 703 people have joined a group and 167 have already reduced the risk of developing Type 2 diabetes.
